I’ve been proudly practicing in South Florida since 2007, always striving to provide a personalized, patient-driven experience. In 2022 I was thrilled to expand my practice to the beautiful Blue Ridge Mountains. Ensuring that women have access to the quality medical care they deserve is deeply important to me. I’m honored to join the High Country community and offer a modern, compassionate alternative for women’s healthcare.
Born and raised in South Florida, I’ve been married since 2003 and am the proud parent of two wonderful boys. Finding the right balance between my work and family life has always been a top priority. I cherish the time I spend with both my patients and my family, and bring that same sense of care and balance to my medical practice.

Sarah J. O'Neal, MS, MPAS, PA-C
Sarah is a Certified Physician Assistant (PA-C) with experience in family medicine, women’s health, and gynecology. Sarah graduated from the University of Kentucky with a Masters of Kinesiology in 2008 and a Masters of Physician Assistant Studies in 2011. She has practiced clinically in Boone and the surrounding high country since 2014. Sarah is passionate helping women and tailoring care to their specific needs.
Nicole (Nikki) Chappell, WHNP-BC, C-RHI
Nikki grew up in the scenic high country of North Carolina. With more than thirty years of experience in women's health, she has dedicated her career to providing individualized, quality care. Her expertise covers a wide range of services, including routine gynecological care, thorough fertility assessments, menopause management, and contraceptive counseling. Nikki is deeply committed to delivering care that follows established guidelines, all within a welcoming and comfortable environment. She prioritizes listening to each patient's individual questions and concerns to ensure they receive the attention they deserve.
Nikki believes that every woman is unique, and this individuality should be recognized in healthcare. She is passionate about dedicating time to each patient, allowing her to provide the highest quality of care possible.
